Covid-19 Screening Document

All parents and players are reminded that each and every time they attend any of the arenas, whether at Tilbury, Wheatley, Memorial or Erickson they need to present the COVID 19 Chatham Health Screening Document.

The form can be downloaded by clicking hereor by going to the "Forms" section on the right hand side of the home page. Once at the arena you will be required to present the form and sign an OHF Screening Document.
These documents are intended to keep us all safe.
The Municipality is intending on integrating a Health Screening App on your phone which will eliminate the need for the one document but till such time as it is approved please continue to use the Screening Document.
 
No form, no entry.
